CENTRAL MD SOCCER ASSOCIATION			2008 PHOTO ID SESSSIONS

Any program or team can arrange to have players' photos taken for their required identification cars. To have a photographer come to your facility, your club or team must have a minimum of ten (10) players, and your facility must have electricity. To make an appointment for a photographer to come to your facility, call Dave Augustyniak, 410-574-1799, or Robin Dietzway, (443) 831-8074 (after 3 pm).

ANYONE NEEDING ID MUST:		BE PRESENT HAVE COPY OF BIRTH CERTIFICATE AND HIS/HER EXPIRED CMSA OR SAY CARD HAVE $5.00 IN CASH BE ACCOMPANIED BY PARENT IF UNDER 18 OPTIONAL: IF THE PLAYER HAS A CURRENT ID OF HIMSELF OR HERSELF, THAT PICTURE CAN BE CUT AND PUT ONTO THE PHOTO ID CARD, BRING THAT PHOTO, IF DESIRED, AS THIS MAY SPEED UP THE PROCESS.
